If you consistently apply ETTR then the midtones can be both over as well as underexposed. It depends on the content of the scene and the dynamic range of the camera.
What I think would be useful is to have an auto gamma (p)redistribution in ACR which moves the middle gray back to 18%. (or whatever the value is for any particular camera/meter).
This can be done because ACR can simulate metering on the linear data, and/or you can use a pipette to select a spot for spot-metering!
