I get the same thing - had been surprised that i could only find this one reference to the problem, which implies that it's only a couple of people who have it -
I convert a raw image via camera raw in either bridge or PS, and the result is a file that's fine except that the thumbnail preview is either solid black or really pixelated (say 15x15). I then have to run the files through an action (in PS) which simply saves the files again, and this produces a file with the a working thumbnail.
Any ideas anyone as it's starting to **** me off!

EDIT: saving files as TIFF with LZW, or PS or JPEG doesn't make any difference, the problem remains
