The AGS Webservice log is at:

However, it may not have any more info since it's an "unknown exception". Temporarily set the WebService logLevel to 7 and see if you get more details.
If you can identify specific PDFs that this happens with, it's usually easiest to run your XML commands against the PDF from the Command line with a logLevel of 7. That way you can watch: each command execute, see how long it takes, and how much RAM it is using (Windows Task Manager).
You may also get useful info if you manually go through your command workflow using Acrobat 5. (That's the PDF engine that AGS uses.)
Preflighting the PDF in Acrobat 6 or 7 may also provide useful info.
- the PDF may be damaged
- the PDF may not be to spec
- the PDF may need more RAM than what's available
- ???
