Is this a Distiller question or a problem you are seeing when using Adobe Document Server?
If the problem occurs when using Distiller, try posting your question on one of the Acrobat forums or contact Acrobat support.
If the problem occurs only when using Adobe Document Server, I'd contact ADS support and provide them with your source file, your XML commands, and the problem output file.
